second thoughts is ok at some level.. it depends if these thoughts are because of u being unsure she is the one.. or bc of you being scared about marriage. if its the first then wait until its more clear to u. if its the second then imagine your life without her and see if you think that is ok.. if not then pop that question!

Do you guys have kids together?

That is great that you are being a father figure to her son and she a mother figure to your daughter.Worrying about her saying no and about the ring should be the last to make you have 2nd thoughts there is SOOO much more to marriage than that.That is just the icing on the cake.My best advice will to be to wait a lil longer think it over more.Don't rush into this.How old is Hottie?
